Description

Trilostane is an FDA approved medication used to treat Cushing’s disease (hyperadrenocorticism) in dogs and cats, and Alopecia X in dogs. It decreases the production of the cortisol hormone by blocking the enzyme needed to make it in the adrenal glands. Trilostane is a synthetic enzyme drug that does not cure the condition but can help manage it.
Dogs usually respond to this medication therapy within two to four weeks. Your veterinarian will monitor your dog closely, especially at the start of the treatment.
Trilostane is also used to treat Cushing’s disease in cats. While this disease is not common in cats, those with Cushing’s disease may also have underlying diabetes mellitus. Trilostane has not been proven to change their insulin needs.
Trilostane can also be used for the treatment of pituitary pars intermedia dysfunction (PPID) in horses.

Uses

Trilostane compounded in cod liver oil is given by mouth in liquid form daily with food for improved bioavailability. This medication should start to take effect within 1 to 2 days.
This medication should be given just as your veterinarian prescribes. It must be measured carefully, and it's best to use the syringe provided to ensure you are able to give the proper dosage required. After dosing your pet, rinse and dry the syringe, and screw the cap tightly onto the bottle. We recommend washing hands after administering the medication. Possible Side Effects

Trilostane compounded in cod liver oil is generally safe, however, the most common side effects can include diarrhea, vomiting, lethargy, mild electrolyte abnormalities, or lack of appetite during the first few days of therapy secondary to steroid withdrawal. These side effects are usually mild.
Hypoadrenocorticism can happen and is generally reversible once the medication is stopped, however in some cases, adrenal gland damage can occur in dogs.
In cats, side effects can include anorexia, lethargy, and dulled mental activity.
A life-threatening condition, called an Addisonian crisis, can occur while your pet is using this medication. Your pet should be monitored closely for diarrhea, vomiting, lethargy, shaking, weakness, collapse, and increased drinking or urination. Contact your veterinarian immediately if these side effects or allergic reactions continue or become more severe.

Drug & Food Interactions

Drug Interactions: If your pet is on any of the following medications you should use trilostane with caution:

  • Ace Inhibitors :(e.g. benazepril, enalapril): When combined may increase risk for hyperkalemia.
  • Ketoconazole: When combined may potentiate the effects of trilostane and lead to hypoadrenocorticism.
  • Mitotane:When combined may potentiate the effects of trilostane and lead to hypoadrenocorticism. Wait at least 1 month after discontinuing mitotane before initiating trilostane.
  • Potassium-Sparing Diuretics(e.g. spironolactone): When combined may increase risk for hyperkalemia.
  • Potassium-Supplements: High Potassium Foods: When combined may increase risk for hyperkalemia.

Precautions

Trilostane should not be given to pets that are hypersensitive or allergic to it. It should not be given to animals that are pregnant. Trilostane is metabolized by the liver and should be used with caution in pets with kidney or liver disease, in animals that are anemic, or in animals that are nursing.
Trilostane can affect the levels of other hormones produced in the adrenal gland. Some dogs can be at an increased risk for metabolic problems including dehydration, weakness, and abnormal serum electrolyte levels.
Keep this medication out of reach of children. Trilostane is a prescription medication and should be used based on your veterinarian’s directions.

Monitoring
